Back Injuries: How to Effectively Address Them
Back pain is one of the most frequent reasons for consultations in any clinic. Even so, what lies behind it is rarely explained well. There is a tendency to simplify it, give it a quick name, and focus everything on a single specific area, but the reality is that the back does not work that way.
The spine is not just a structure. It is a system that needs to move, adapt, and constantly support loads. When that balance is disrupted, symptoms begin to appear—sometimes suddenly, other times progressively—until the pain settles in and begins to condition daily activity.
At that point, it is common to look for a single specific cause. Low back pain, a herniated disc, a muscle contracture… but often what we find is something more complex: a combination of factors that have accumulated over time.
For this reason, when we talk about back injuries, we are not referring exclusively to a specific diagnosis. We include conditions such as low back pain (lumbalgia), sciatica (lumbociatalgia), neck pain (cervicalgia), or upper back pain (dorsalgia)—as we explain in detail in our article on common spine injuries—which, although they manifest differently, usually share a common foundation related to how the spine is functioning.

It is common, limiting, and causes concern, but it does not always stem from a clear structural injury. In fact, in many cases, what appears is a loss of the area’s capacity to tolerate load and movement. This explains why prolonged rest is usually not a solution and why the problem tends to recur if not addressed correctly.
When pain radiates down the leg, as occurs in sciatica (lumbociatalgia)—which we discuss in detail here—the situation changes in terms of symptoms, but not so much in terms of approach. It remains fundamental to understand how the tissue is responding and what capacity it has to recover.
Something similar happens with herniated discs. Although they tend to cause more alarm, they do not always imply a permanent limitation nor do they require invasive interventions. In other areas of the back, such as the cervical or thoracic region, the context is usually different, but the origin of the problem once again relates to a lack of movement, sustained loads, or repeated patterns over time. It is no coincidence that these pains frequently appear in sedentary work environments or during prolonged periods of stress.
One of the most common mistakes is interpreting the back solely based on imaging tests. However, in clinical practice, there is not always a direct relationship between what is seen and what is felt. There are people with significant structural changes who barely show symptoms, while others, with minor imaging findings, experience persistent pain. This forces a shift in approach.
Treatment cannot focus solely on structure, but rather on function: how that back moves, what capacity it has to bear loads, and what it needs to adapt better.
From there, the approach is built progressively. Restoring movement is one of the first key steps because stiffness and lack of mobility tend to perpetuate the problem. Added to this is therapeutic exercise, which improves strength, stability, and effort tolerance. In most cases, it is the element that makes the real difference in recovery.

Understanding this completely changes how back injuries are approached. It is not just about identifying what hurts, but analyzing why it stopped functioning well and what it needs to work properly again. That is where true recovery begins.
